UNICARE APPLICATION OPTIONS:

Once your application has been approved, Unicare will charge your credit card for the 1st month premium.

AETNA APPLICATION OPTIONS

EMAIL - Use our forms page to download an Aetna application, or upon your request we can email one to you for you to complete and email back to us. We will submit it to Aetna. The usual processing time is 4-7 days. The first month premium must be paid by check made payable to AETNA. HUMANA APPLICATION OPTIONS

TELEPHONE - We can take your information for your application over the phone. We will submit it to Humana. The usual processing time is 5-10 days. The first months premium will be debited from the credit card or checking account of your choice upon approval.

Step 3:

After your application has been submitted, we will keep in touch with you by phone and or email to keep you updated on the status of your application. We have our own customer administration representative who only works on applications and getting them processed quickly.
